Because there are more risks with having a total knee replacement in both legs at the same time than having a knee replacement in one leg, doctors in recent years have been selecting younger and healthier patients for the bilateral procedure. Now a new study by researchers at Hospital for Special Surgery has revealed that although patients are younger and healthier than those undergoing only one-sided surgery, they are becoming sicker and some complication rates have risen.
"Although we are selecting younger people, we can't ignore the fact that the population in general is getting sicker," said Stavros Memtsoudis, M.D., Ph.D., director of Critical Care Services at Hospital for Special Surgery (HSS) in New York City, who led the study. "Many of the complications that we studied didn't decrease over time as you would expect with younger patients and better health care, and some of the complications even increased." The study is published online ahead of print in the journal Clinical Orthopaedics and Related Research.
Rising levels of obesity appear to be driving the complication rates, as well as the increase in the utilization of total knee replacements. Obesity puts extra stress on joints and bones and can cause premature osteoarthritis.
To conduct their research into trends regarding bilateral total knee replacements, researchers at HSS turned to the Nationwide Inpatient Survey, sponsored by the Agency for Healthcare Research and Quality (AHRQ). This is the largest inpatient database available in the United States that includes information on patients of all ages. It collects data from about 20% of all hospitalizations in the U.S. The study investigators identified 258,524 bilateral total knee replacements performed between 1999 and 2008. The number of annual bilateral procedures increased by 75%. In 1999, bilateral procedures accounted for 3.7% of all knee replacement operations and in 2008, they accounted for 6% of the operations.
The average age of patients undergoing bilateral knee replacement decreased by 2.5 years from 1999 to 2008. As the years marched on, patients presented with higher rates of comorbidities, including renal failure, neurologic disease, liver disease, diabetes, and chronic obstructive pulmonary disease. Obesity increased by 131% during the study period. The only comorbidity that decreased was congestive heart failure.
Over the time period studied, the researchers identified a 3% increase in pneumonia, a 6% increase in pulmonary embolism, and a 3% increase in nonmyocardial infarction cardiac complications.
"The take home message of this paper is that we are fighting an uphill battle, because people are getting sicker, despite us attempting to limit risk by choosing younger people to do these procedures in," said Dr. Memtsoudis. "We have to start thinking of other interventions other than patient selection that we can implement in order to reduce these complications."
On a positive note, the investigators also found that absolute in-hospital mortality rates decreased at an average rate of 10% per year. The researchers attribute this decrease to advances in medicine and increased use of telemetry and observation in the last 10 to 15 years. "With advances in medicine and monitoring, we have been able to counteract this extreme event of mortality. If you get a complication and it is not recognized, you may die from it," said Dr. Memtsoudis. "If you observe people more thoroughly, you may not be able to prevent the complication, but you may be able to prevent a mortal event resulting from it."
The researchers also identified a cost shifting landscape. Patients stayed in the hospital an average of five days after their procedure in 1999 and an average of four days in 2008, but the proportion of discharges to a home or customary residence without home health care decreased at an average rate of 5.5% per year.
"Before this study, we were under the assumption that patient complications were steadily decreasing after bilateral knee arthroplasty, because of better patient selection and improvements in medical care," said Dr. Memtsoudis. "Now we understand that the picture is more complex. Patients being selected for the procedure may be getting younger, but they are not getting healthier and maybe that is why we don't see a drastic drop in complications."
Before embarking on a bilateral knee replacement, he said, patients should "critically look at themselves and talk to their physicians about how their health status plays into the choice of surgery."
More work is needed to identify ways to prevent complications in patients undergoing bilateral knee replacement and a recent conference sponsored by the Hospital for Special Surgery is aiming to do just that. The Consensus Conference on the Creation of Guidelines for Bilateral Knee Arthroplasty involved 40 experts from 16 institutions. The guidelines coming out of this conference, which are expected to be published within the next six months, address issues such as determining the appropriate workup and management for a patient undergoing bilateral knee replacement, and how long doctors should wait between procedures if a patient undergoes two operations.

Secondly, ratings are harder to find and many TV stations are struggling to compete just to get the biggest audience share possible. Also, TV at present is undoubtedly not only the place where ads are effectively channeled. And lastly, social media has affected almost every aspect of advertising including standard infomercials. These are but some of the changes that have affected the direct response marketing industry. This means that it is in a tougher time period that faces lots of other competing advertising platforms. In this regard, an infomercial made 15 years back will most likely fail if launched today. This is the reason product demonstrations on TV have been overhauled.
If you have noticed, those who are still watching infomercials today often belong to the 30s, 40s or higher age groups. Kids, teens and particularly the young adults usually do not have the luxury of time and interest to watch these TV ads. It is either they are busy doing their work or school tasks or probably because they don?t use TV that regularly because of too much smart phone and Internet immersion. The Perks of Being a Wallflower may just be one of the best films you see all year, which isn't a surprise considering the author of the novel on which it's based wrote the screenplay and also directed the movie. Having an author adapt his/her novel into a screenplay is common, but to have that author also direct the film is highly unusual, and Stephen Chbosky only had one directing credit to his name (from 1995) before this film. With a talented cast including Emma Watson (sporting an American accent!), Logan Lerman, and Ezra Miller, this movie is one that you can't miss.
The scene is early 1990s Philadelphia, and Logan Lerman plays Charlie, an incoming freshman in high school who has lately been having a tough time. It's alluded that he used to hear voices and "see things," and things got worse after a tragic event involving his best friend in 8th grade. As a 9th grader, he has no friends, even though his sister, Candace (Nina Dobrev), attends the same school as him; she's too busy hanging out with her boyfriend, Ponytail Derek (Nicholas Braun), to introduce Charlie to people. Charlie soon becomes friends with Patrick (Ezra Miller), though, a senior retaking his freshman shop class, and he introduces Charlie to fellow seniors Sam (Emma Watson), Mary Elizabeth (Mae Whitman), and others who aren't afraid to "be different."
Charlie has a major crush on Sam, who ends up being his first kiss; after going to the Sadie Hawkins dance with Mary Elizabeth, however, he finds himself suddenly dating her, which he's not really happy about. Sam is dating an older college guy, meanwhile, and Charlie is afraid to tell her about his feelings. Patrick is secretly dating the school quarterback, because the quarterback has a girlfriend and doesn't want to come out of the closet to his friends. Charlie must navigate through freshman year of high school, but soon finds that life is much happier when you have friends to share it with.
Most people know Logan Lerman from the Percy Jackson and the Olympians series, and I thought he did extraordinarily well here; so did Watson, as his friend Sam, and Miller, as the gay artistic friend. Paul Rudd has a small role as Charlie's English teacher, whom Charlie forms a bond with since he loves to read, and Dylan McDermott and Kate Walsh play Charlie's mostly-normal parents. Melanie Lynskey also appears in flashbacks, as Charlie's deceased aunt with whom he shared a secret. I was also pleasantly surprised at Watson's American accent; if you had never seen her in the Harry Potter series, you would have definitely believed she was American, and as the movie went on I began to forget that she used to be Hermione in those films.
Yes, definitely see this movie. I'm guessing fans of the book will love this film as well, though I should reread the novel at some point to compare the two. Everything that happens in the movie could have happened in a real high school, and the last 10 or 15 minutes puts an interesting spin on the film's plot as a whole. I actually wouldn't even mind a sequel to Wallflower, though that won't happen, as I found myself thinking about Charlie long after the movie ended and wondering how he would fare in his sophomore year and beyond. Most people hated high school, yet this film makes you want to relive those days - the friends, the experiences, the ups and downs, and, as Charlie says, the feeling of "being infinite."
The Perks of Being a Wallflower is in theaters now and is rated PG-13 with a runtime of 103 minutes. 4.5 stars out of 5.

RIO DE JANEIRO (AP) ? The arrest of a top Google executive is reviving a debate about Brazilian laws that hold services such as YouTube responsible for the videos posted on them, making the country a hotbed of attempts to stifle digital content.
Legal experts said Thursday that Google violated a judge's order to take down videos on its YouTube subsidiary that target Brazilian political candidates ? and that the judge was completely within the law in issuing the arrest warrant.
But they said the arrest of Fabio Jose Silva Coelho, the head of Google Inc.'s Brazil operations, underscores the need to modernize laws that treat offensive material on the Internet like material that is carried by newspapers, television and radio, holding platforms such as Google responsible for user-provided content.
Coelho was released shortly after his arrest Wednesday and agreed to appear before a court at an as-yet undetermined time. On Google's official Brazil blog, Coelho wrote Thursday night that the company was forced to block the video in the case for which he was arrested after the company lost its final appeal.
"We are deeply disappointed that we have never had the full opportunity to argue in court that these were legitimate free speech videos and should remain available in Brazil," he wrote. "Despite all this, we will continue to campaign for free expression globally."
Legal experts said the case cast a spotlight on problems within Brazil's legal system.
"Our laws trying to govern the Internet are outdated," said Jose Guilherme Zagallo, head of the Brazilian bar association's commission focusing on information technology law. "It's not clear who is responsible for content, and that creates uncertainty for Internet companies, users and judges, who are left to interpret laws not written for the Internet."
Brazil's strict electoral laws limit what critics can say on television, radio and the Internet about candidates for office. On several occasions in recent years, media outlets have faced stiff fines for breaking the laws, but few if any officials were arrested.
Google's alleged infractions, however, are more widespread, simply because of its omnipresence. Ahead of municipal elections in Brazil next month, Google has received requests in more than 20 states to remove videos that allegedly violate those restrictions.
Google has faced a landslide of content-removal requests around the globe, including in the U.S., but Brazil makes more requests than any other nation, according to the company's summary of all the demands. Most such demands relate to legitimate attempts to enforce laws on issues ranging from personal privacy to hate speech.
Brazilian government agencies alone submitted a total of 194 content-removal requests during the final half of last year, according to a summary released by Google in June. Running just behind that was the United States, where police, prosecutors, courts and other government agencies submitted 187 requests to remove content over the same period.
Google says it complied fully or partially with 54 percent of Brazilian removal requests in the last half of 2011. Most requests involved YouTube and charges of defamation. Other requests involved the social networking site Orkut and requests to remove illegal content, such as child pornography.
Separately this week, another Brazilian court ordered YouTube to remove clips of an anti-Islam film that has been blamed for deadly protests by Muslims around the globe. Google is now selectively blocking the video clips in countries that include Libya and Egypt. Google has said it made the decision to block the video in such places due to "the sensitive situations" there.
Brazil's legal action targeting a Google executive, while rare, is not unprecedented. In 2010 in Italy, a judge held three Google executives criminally responsible for an online video of an autistic teenager being bullied. The executives were given six-month suspended sentences.
A judge in Mato Grosso do Sul state ordered Coelho arrested because the company had not removed YouTube videos making incendiary comments about an alleged paternity suit aimed at Alcides Bernal, who is running for mayor of the city of Campo Grande.
"Being a platform, Google is not responsible for the content posted on its site," the company said in a statement this week.
Bruno Magrani, a researcher at the Center for Technology and Society at Rio de Janeiro's Getulio Vargas Foundation, said that unlike the United States and some other countries, Brazil doesn't have legal protections for online service providers that host content provided by third parties.
There is pending legislation that would provide some protection for intermediaries such as Google. Earlier this month the company joined Facebook and online retail site MercadoLivre in sending an open letter supporting the passage of the law, called Marco Civil.
"Marco Civil establishes that providers of Internet applications are not responsible for content published by users," the letter says. "Various economic, social and legislative factors justify not holding providers responsible; without that protection, the use of online applications and platforms would be limited, which would be a loss to users."
While the measure would create some protections, it would not resolve the legal tangle facing Google's Coelho or prevent the situation from recurring, Magrani said.
The Marco Civil is general legislation, and could still be trumped by more specific electoral laws. Those laws treat an Internet platform such as Google as if it were a newspaper or a television station, holding it responsible for its content.
"It's a very serious situation," Magrani said. "Brazil needs to change its electoral law to accommodate the nature and the characteristics of the Internet. The Internet cannot be treated in the same way as traditional media."
First, he said, an Internet company cannot evaluate all the content it carries in the same way a newspaper or television channel can because of the sheer volume.
Second, "the Internet has no editor. And we don't want an editor," Magrani said.
He said asking a company to determine what users can upload is a dangerous step that could undermine freedom of information.
"If we continue threatening to jail heads of companies who don't verify content before it goes on the Internet, we will end up living in a state of censorship," he said. "If the company is running a high risk, it'll start posting less and less material. ... If companies start to feel afraid of retaliation, they'll start censoring."
The lack of protections for Internet platforms can also have a chilling effect on the development of small- and medium-size high-tech companies in Brazil that don't have the resources of big companies like Google, Magrani said.
The federal government is investing heavily to promote the tech sector, but Brazilian legislators need to diminish legal risks for startups, he said.
Maria Clara Garcaz, a 20-year-old university student in Rio de Janeiro, expressed worries about the court action.
"It's like we live in a silent, disguised dictatorship. When we had our real dictatorship, at least you knew for certain what you could and couldn't say," Garcaz said. "Political speech can be censored at any time and it's moving into the Internet, exactly where people speak out."

Japanese Prime Minister Yoshihiko Noda said in New York a day earlier that the islands are clearly an "inherent part of our territory, in light of history and international law." He said that issues over the islands should be resolved peacefully and by the rule of law.
"China is strongly disappointed and sternly opposes the Japanese leader's obstinacy regarding his wrong position" on the matter, Foreign Ministry spokesman Qin Gang said in a statement that repeated China's stance that Japan was ignoring historical facts and international laws.
"The country seriously challenges the post-war international order, but tries to take the rules of international law as a cover. This is self-deceiving," Qin said in a separate statement.
Senior diplomats from both countries have met this week in New York and Beijing in an attempt to mend ties frayed by the spat over the island group in the East China Sea known as Senkaku in Japan and Diaoyu in China.
The islands, held by Japan, are uninhabited but sit astride rich fishing waters and potentially large reserves of natural gas. They are also claimed by Taiwan.
Japan's purchase of some of the islands from their private Japanese owners two weeks ago sparked sometimes violent protests in China that targeted Japanese-owned stores and factories.
Noda defended the purchase as an attempt to ensure their "stable management," but conceded "it seems that China has yet to understand that."

Let me explain. About 21 years ago, my wife and I were expecting our second child. It was a long-awaited sibling to our five-year old daughter. The pregnancy was normal, but the joy of a new baby suddenly turned into sorrow. Three days before the birth, the midwife could no longer hear the baby?s heartbeat. It was very difficult to understand the tragedy that had hit us so suddenly.?We got the advice from both the midwife and the doctor to give birth to the child as normal as possible. They asked us to go home over night, and return the next morning when we would meet the same staff again. We went home, called our relatives and friends, and told them that our new child was dead. We were in a state of chock.? A few hours later my wife suddenly went into labor, and we had to rush back to the hospital.
Back at the hospital, the midwife said to my wife ?Now you are going to give birth?. These words made us forget all about our worries, and turned our focus on being present in the moment. The birth itself was beautiful, one of the most beautiful moments in my life. My wife was breathing. I was breathing and the staff around us was breathing. However, my son Anton was stillborn. He was not breathing, or crying. The umbilical cord was wrapped around his neck four times, and it also had a knot on it.?We stayed over night, and the staff was incredibly supportive. Slowly we started to realize our loss. We took a lot of photos of Anton, cut off a piece of his hair to keep as a memory. They recommended us to give Anton a bath, and we did. Later I have learned that this practise helped us to sense the beauty of our beloved son with all our senses. This was a way to create memories, and to start the process of grief recovery.
That day and night when Anton was born, I was in deep sorrow.? The loss of a future, a future with my living son was enormous. But that night when I tried to go to sleep, I was also happy. I was as happy as any father to a new-born baby. Happiness mixed with grief. Thank you Karin, my wife, for all your support. Thank you Elin, for showing me how natural a child can handle grief. WAIK?K? ? Twenty years ago, with the help of many concerned residents and businesses, the Waik?k? Improvement Association (WIA) held the inaugural Waik?k? 20/20 Conference to plan for the future of Waik?k?. This year, more than 25 speakers from the public and private sectors of Hawaii?s business communities will gather to review what has been accomplished since 1992 and to engage the community in conversation to plan for the next 20 years of Waik?k?. The Waik?k? 20/20 Conference will be held on Tuesday, October 16 from 8 a.m. to 3:30 p.m. at the Hawai?i Convention Center, Lili?u Theatre.
?We?ve come along way since 20 years ago and we would like to continue the discussion and re-engage the community about Waik?k??s future, with the goal of developing a working plan in the six to eight months following the conference,? said Rick Egged, president of the WIA. ?This is an important effort that will help make Waik?k? more livable and more attractive to residents and visitors for years to come.?
A range of experts from different industries will make presentations on topics and issues that most affect Waik?k? and the surrounding communities for the next generation of residents and visitors. Topics and issues include Waik?k? Beach nourishment program; upgrading of Waik?k? amenities and common areas; residential development and challenges; future expansions in Waik?k??s retail market; transportation; energy and sustainability; public health and safety including homelessness and crime. A roundtable discussion by young professionals to get their thoughts and views on the topics and issues will be presented at the conference.
Monthly forums will be held in the six to eight months following the conference to recap what was discussed in each session and to develop a plan that will be implemented for the next 20 years. Dates, times and locations of each monthly forum will be announced at a later time. The WIA Annual Meeting will also follow the Waik?k? 20/20 Conference on the same day from 4 p.m. to 7:30 p.m. and will include an awards ceremony and reception.

Paris ? the last city after New York, London and Milan to host shows ? is offering up 95 frenetic catwalk spectacles back-to-back over eight days at opposite ends of the French capital.
So it's perhaps fitting that the spring-summer 2013 season got going behind a giant clock: The iconic timekeeper of Paris' grand Gare de l'Est train station. The setting may have been grand but the shows on the first day in Paris are often low-key, a showcase for emerging talent.
Young knitwear designer and Rick Owens alumnus Alice Lemoine emerged from the shadows of her former mentor to produce an unassuming but highly accomplished show in rich spring colors.
Belgian-born designer Anthony Vaccarello ? whose star has been on a rise since Gwyneth Paltrow graced the cover of Harper's Bazaar in one of his dresses ? delivered a slick and revealing show Tuesday, channeling black and white in an unusual summer collection.
South Korean designer Moon Young Hee also threw away the color wheel to produce a sophisticated, demure show, while Impasse de la Defense mixed bold colors with retro and street styles.
Wednesday's shows will include London's enfant terrible Gareth Pugh, Guy Laroche and Dries Van Noten.
IMPASSE DE LA DEFENSE
Post-punk design house Impasse de la Defense, tucked away behind the clock of the Gare de l'Est, got extra street cred as loud train announcements punctuated their colorful show.
Their eclectic and contemporary mix included vibrant patchwork dresses, outre tulle bridal skirts and large shawls printed with images of clock architecture. Their soundtrack ? a single harmonica played by a man who looked like a busker ? added a dash of boho insouciance to the many the casual, loose-fitting, splash-dyed dresses.
Designer Karim Bonnet said he was channeling German street style after a holiday there last summer ? but at times, the clock seemed to turn to '70s flower power. The imaginative prints of cameras and the spines of novels on large shawls were a notable success.
At times, though, the sheer size of the shawls ? which models had wrapped around them ? made them look clumsy.
LE MOINE TRICOTE
No longer just for grannies, knitwear has finally been made cool ? a youthful facelift courtesy of Le Moine Tricote.
Armed with two 12mm (half-inch) needles, a ball of wool and no rules, designer Alice Lemoine set about this collection with no idea what clothes she would make.
"I just let the needles lead and I make all sorts of different shapes and panels," Lemoine said backstage, wearing a wooly cardigan. "I then just fuse it together; not exactly patchwork, but the same process."
The result? An architectural ? and highly huggable ? display of some 14 soft, open knit looks ? set off with creative spirals, polygons and geometric shapes.
Lemoine gives freedom to the natural weight of wool ? producing some sumptuous effects, like one plunging gray and camel column dress. Lest we forget it's summer, it's all served up with a refreshing palette of sky blue, navy, pearl beige purple and white.
Splashes of delicate, tightly knitted silk alongside breezy organza undergarments and the occasional peplum was a new addition this season.
ANTHONY VACCARELLO
